About My Hakeem Platform
“Your gateway to your health record”
“MyHakeem” is a web and mobile based service that enables its users to access and view their pre-booked medical appointments, also selective medical information such as; active and pending medications, recent immunizations, pending and complete laboratory tests, allergies and vital signs that are stored on the user’s electronic health record registered within any healthcare facility that implements “Hakeem program”. It also enables the user to request the delivery of his/her monthly medication prescription refills.
You can benefit from “MyHakeem” services from anywhere and at any time by accessing MyHakeem website or downloading My Hakeem application.
Users can access My Hakeem mobile application or MyHakeem.jo platform by using the same log in credentials.
“My Hakeem” platform is free of charge, and is provided by Electronic Health Solutions Company (referred to later as The Company), in order to enable users to register on the platform and benefit from the electronic services.
“My Hakeem” platform is a web and a mobile app service.
The user is held accountable for sharing his/her National ID / Medical No. and password with others.
“My Hakeem” platform is only an information providing tool that delivers the saved information available on the “Hakeem electronic medical records database”.
“My Hakeem” platform is not responsible for any loss, damage or disclosure of the users’ personal information that might occur due to any misuse of the platform.
The information resulting from using “My Hakeem” platform will be used to serve Electronic Health Solutions (a non-profit private company) for statistics and/or supporting legal obligations only and will not be used for any other reason no matter what, and the company will dismiss them after a certain period of time.
“My Hakeem” platform will not use or share any of the users’ private information with any person/authority unless consistent with the terms of use.
“My Hakeem” platform provides the monthly medication ordering service for the user on the approved dates for dispensing these medicines through a delivery company.
The User acknowledges that the payment process of medication delivery will take place through a certain application which is independent of “My Hakeem” platform. Where the user knows that “My Hakeem” platform is not responsible for any financial costs that will be deducted from him for the purposes of performing this service. In addition “My Hakeem” platform is not responsible for any of the problems that the user may encounter in the process of delivering medicines for any reason.
“My Hakeem” platform will not request any financial transactions and records under any condition.
“My Hakeem” platform is owned by Electronic Health Solutions Company; a non-profit private company, and any attempt, in any way, to hack into the platform will be logged and the company will take legal actions.
The Company has the right to immediately suspend/close any account on “My Hakeem” platform for misuse or violation of the terms of use and without notice.
The user is solely responsible for the information provided on “My Hakeem” platform.
In case of any dispute and/or conflict over implementing and/or interpreting any article hereof, the Justice Palace Court (in Amman) will be the Court competent to look into such dispute, and the Jordanian Law will be the applicable Law.
“My Hakeem” platform requires a first time registry in The Hashemite Kingdom of Jordan therefore, it is ruled by its legal procedures and conventions.
In case of any discrepancy between the Arabic and the English language, the English version shall prevail.
You are solely responsible for any damage, loss or breach that may result due to breaking these “Terms of Use” and/or practicing any of the prohibited activities referred to above, and acknowledge that Electronic Health Solutions Company assumes no liability to you or any other party for any loss.

Make sure your insurance is still valid and have your medical reports ready before requesting the medication delivery service, furthermore please check your medications and remaining refills at “Requests log” menu.
Make sure the phone number you provided is correct.
Make sure to read the notes and enter all requested details correctly in order for your request to be processed successfully.
Only request your monthly medication refills, as you are not entitled for a refill if the duration from the last refill has not passed the specified time per health insurance regulations.
Ensure to only enter the name of the hospital/health clinic of which your last medication refill was dispensed from in order for the submitted request to be approved.
The Ministry of Health personnel at your healthcare facility will follow up on all submitted requests to complete and manage the delivery of your medication.
Enter the correct and full Address of your residence. Any incorrect information provided might cause your request to be cancelled with no refunds.
For any inquiries regarding your medical prescription, kindly contact your healthcare facility.
Make sure to interact with the notifications/ messages sent to you regarding the medication payment and delivery within the given time limit, any failure to do so might cause your request to be cancelled with no refunds.
The patient or someone on his/her behalf (must be 18 years or older) must show an ID to the delivery agent upon delivery and receipt of the medications and sign the appropriate forms provided.
This service does not include medical prescriptions issued from healthcare facilities that are not included in the Hakeem network.
